Enhanced Dementia Care Service

Different organisations in Hounslow are working together to provide an Enhanced Dementia Care Service.

By working together, we aim to co-ordinate care and give support to older people living with dementia and their carers.  We share a common goal to collaborate with clients and carers, to provide the right care for your specific needs.

How will the service help you?

If you are living with dementia, this new service will help ensure you receive the appropriate health and social care support to maximise your independence and wellbeing.

This service will co-ordinate with other care providers, hospitals, community mental health teams, charities and community health services. The service is staffed by a clinical nurse and dementia co-ordinators from a range of backgrounds. You will have a named co-ordinator who will provide information, advice and support to you and your family. 

Your Dementia Co-ordinator will:

  • Advise and support you and your carer about managing your dementia.
  • Help you to access additional support in your home, to give your carer a break, and advise on activities and social opportunities you may enjoy.
  • Assist with co-ordinating your care.
  • Monitor general health and wellbeing.
  • Develop a personalised care plan with you and your carer. The care plan will be reviewed with you regularly to make sure we are delivering the highest possible standard of care and helping you to remain safely at home for as long as possible.
  • Support you to make plans for the future.

How do you access the service?

The team is working closely with GPs, community services and dementia services to identify appropriate people for the service. At this stage the service does not accept referrals, but you can speak to us directly for us to ascertain your suitability for this service or request contact be made to us via one of your care providers.

This is a new service, designed to provide appropriate care for older people who might need extra support to stay well. We hope to extend the service to more people over time.
