The Children's Continuing Care Service provides healthcare for children who have a range of conditions that would otherwise need hospital treatment or residential care.
The team consists of experienced children’s nurses who provide an assessment service for those children who are referred to children’s continuing care. The child is assessed using the National Framework for Children and Young People’s Continuing Care, March 2010.
Following agreement of their eligibility at the Joint Funding Panel, a care package is sourced/provided and monitored by the team. Following the initial assessment if the child meets the criteria, the child’s care package will then be reviewed every 3-12 months according to the level of need.
