All referrals to the service from GPs or other health and social care professionals should be made using the HRCH single referral form (Hounslow) via the trust's Single Point of Access (Hounslow) service.
The service appreciates the inclusion of discharge summaries, clinic letters and reports as appropriate.
Any health and social care professional or voluntary agency can refer people to this service.

Be registered with a Hounslow GP or resident in London Borough of Hounslow

Require rehabilitation and reablement after an acute illness, injury or change in life circumstances with a primary physical or social need.
