The Richmond Response and Rehabilitation Team (RRRT) are holding two open days for local GPs and healthcare professionals to find out more about the innovative service which provides seemless integrated care, support and rehabilitation to local people in Richmond.

About the open-house events:
The events will be your chance to find out more about the work of the team and how they can make a difference to your practice and patients. You can also find out about the types of patients that are suitable to be referred and how you can access and refer your patients to this service.
Service leads will be on hand at the event to listen to your views and feedback about how we can tailor the provision to your patient’s needs; in addition to capturing any issues or problems that GPs and healthcare professionals may have encountered.
About the Richmond Response and Rehabilitation Team:
The RRRT is an integrated health and social care service for adults, primarily older people, which is commissioned by Richmond CCG and delivered in partnership between Hounslow and Richmond Community Healthcare NHS Trust and the London Borough of Richmond upon Thames.
The service brings together a range of health and social care professionals to provide intensive rehabilitation and practical support following a period of illness, disability or following hospital discharge. The team also provides a rapid response service to manage crisis and support people to stay at home, preventing unnecessary admission to an acute hospital or a residential/nursing home.
