Innovative Hounslow health service shortlisted for two HSJ Awards

A local NHS service in Hounslow which seeks to prevent people being admitted to hospital if they don’t need to be - or to get back home more quickly if they do – has been nominated for two prestigious healthcare awards in recognition of the innovative care they provide. 

The Integrated Community Response Service, which is provided by Hounslow and Richmond Community Healthcare NHS Trust, has been shortlisted for the Health Service Journal (HSJ) awards in the categories of: ‘Value and improvement in specialist services, and ‘Value and improvement in community health design’.   

The team was shortlisted in recognition of their innovative service model and clinical initiatives which alleviate the pressure on acute hospitals and the wider health system in Hounslow - and help patients to recover more effectively in the comfort of their own homes, instead of in hospital.  

In 2014, the team also launched a fast-track referral system with the London Ambulance Service (LAS) which allows the LAS to make telephone referrals to the team with an agreed two hour response time, reducing the need to transport vulnerable patients to A&E by providing medical and therapeutic care and expertise to the patient in their own home.

The multidisciplinary team of healthcare experts is made up of a GP, nurses, occupational therapists, physiotherapists, support staff, social workers, primary care mental health nurse and a handyman – who work together to ensure patients receive the right care from the right people at the right time.

Frank Sims, chief executive of Hounslow and Richmond Community Healthcare NHS Trust said: “I am incredibly proud that one of our services has received such fantastic recognition for their quality of care, innovation and outcomes they have been achieving. The team delivers a vital service for local people in Hounslow which alleviates the burden on local acute hospitals and the ambulance service through effective, joined-up working.  

“I have been informed that the standard of competition this year was extremely high and that judges have commended the high quality of entries - so to be shortlisted is a fantastic achievement for one of our services.”

The next stage of the competition will involve the team presenting their entry to a national judging panel in early June - made up of senior and influential figures from the health sector – before the winners are announced at a prestigious awards ceremony later in the year.
