Supporting independence and helping people recover after an illness, injury or a change in life circumstances

The trust has launched a new health and social care rehabilitation service for Hounslow adults to help people recover and regain their independence after a serious illness, injury or change in life circumstances - including neurological conditions such as a stroke, multiple sclerosis, or Parkinson's.
The new Hounslow Community Recovery Service is a partnership initiative between Hounslow and Richmond Community Healthcare NHS Trust (HRCH) and Hounslow Council.
It brings together therapists, specialist nurses, and rehabilitation assistants from HRCH NHS Trust - together with social workers, occupational therapists and support workers from Hounslow Council into a single, easy to access service.
By basing different health and social care specialists from the local NHS trust and council into a single, easy to access service - the team is able to provide more coordinated and responsive care for local people to help them recover more quickly and effectively following illness or injury.
The new service aims to be truly responsive to patient’s needs - operating seven days a week, 365 days a year, between 8am to 8pm to ensure patients get the right support and care at the right time to improve their rehabilitation. Care and support from the team is provided to patients in their own homes or in other local community locations.
When people are referred to the service, the team make contact the person or carer within 24 hours, before an assessment and personalised ‘care plan’ is put in place. Members of the team work with the patient, and/or their families or carers, to develop the ‘care plan’ – which sets out how the team will help the patient regain their daily living skills during their rehabilitation period.
Jacki Hunt, clinical services manager at Hounslow and Richmond Community Healthcare NHS Trust said: “This exciting new service will improve local rehabilitation and care services for Hounslow adults – providing patients with a single access point to a range of health and social care professionals and services that may be required as part of a person’s recovery.
“The service ensures the patient is able to access the right care at the right time from the appropriate NHS specialist or social care workers, helping patients to regain their independence as quickly and effectively as possible and keeping people safe.”
Who can use the service?
In order to qualify for the service, people must be aged 18 or over; registered with a Hounslow GP or be a resident within Hounslow borough and require rehabilitation or reablement after an acute illness, injury or change in life circumstances, with a primary physical or social need.
Referrals can be made by GPs or other healthcare professionals, whilst the service also accepts self-referrals.
